Diabetic Recall Wizard for Vision

This Diabetic Recall Wizard program provides a easy fail safe way to recall
every eligible patient in Vision.

Overview

The idea of this process is to simplify the Diabetic recall procedure within Vision and also to reduce the need to view data within each patient record, as the search criteria should provide enough information to be able to determine those patients that need recalling.

With just a few searches in Vision, all the data is automatically collated in the Diabetic Recall Wizard program and presented in Microsoft Excel.

Once the required recall list of patients have been filtered in Excel, the Diabetic Recall Wizard will automatically create a patient workgroup back into Vision. Whereby the Vision mail merge program is used to produce the letters and forward any recalls in each patient record.

With this process and in addition to the recall procedure:

• No patient should ever be missed from receiving a recall, as it includes all patients with the specific condition

• It will highlight any past history recalls or those that have no recall in place

• Patients that need a 1st, 2nd and 3rd Diabetic recall

We also added a clinical aspect to this Diabetic wizard in that it will also show diagnosis, last hba1c, creatinine, cholesterol, blood pressure, weight, foot care and smoking status. It also includes any QOF exceptions, refusal for review, annual review and monitoring codes.

Once the wizard has written everything into the Microsoft Excel 2003 spreadsheet, it will also highlight any recall and review dates over a year old. The Month and Year columns will show any past history recalls from today in red text and from a clinical perspective...

If a Cholesterol value is:

• between 5 and 6.4 the background cell will turn yellow

• between 6.5 and 7.8 the background cell will turn orange

• greater than 7.8 the background cell will turn red

If HbA1c IFCC (mmol/mol) value is:

• between 37 and 42 the background cell will turn yellow

• between 43 and 57 the background cell will turn orange

• greater than 58 the background cell will turn red

HbA1c IFCC relationship calculation : [DCCT value - 2.15] x 10.929

If Creatinine value is:

• between 0.5 and 50 the background cell will turn orange

• greater than 111 the background cell will turn red

If a Systolic BP value is:

• less than 90 the background cell will turn yellow with red text

• between 140 and 159 the background cell will turn orange

• greater than 160 the background cell will turn red

If a Diastolic BP value is:

• less than 60 the background cell will turn yellow with red text

• between 90 and 99 the background cell will turn orange

• greater than 100 the background cell will turn red

Quick Overview of Recall Procedure

...when used with the Diabetic Recall Wizard:

1. Run two searches in Vision

2. Run Diabetic Recall Wizard to produce data in Microsoft Excel

3. Filter data in Microsoft Excel to define recall group

4. Create Patient Group in Vision using Diabetic Recalls toolbar

5. Use Group Recall screen in Vision to send out the invite letters and add next recall into each patient record.

This program will work on both LAN and Hosted Server sites as there is no installation required.

However, if on a hosted server please make sure you download and run the program within the hosted environment!

Training Day

If required a chargeable training day may be scheduled to demonstrate how to use the wizard, tidy up the recall data and set up the initial patient groups ready for recalls in the future.

Licence Agreement

This program may only be used after the trail period once a practice licence has been purchased from TailorMade Information Technology Solutions Ltd for £25.00+VAT and is subject to the EULA available within the program.

Licence support and further program updates are only valid for 1 year. Thereafter any further program updates and support will require a licence renewal.

Note: You original purchased program may be used indefinitely.
